EMPLOYEE WAS REACHING INTO THE EZ SPRED BALER TO CLEAN SOME CLAY OUT AFTER A BAG BROKE. HE DID NOT TURN THE POWER SWITCH OFF. ANOTHER EMPLOYEE PROCEEDED TO PUT A BAG ON THE END OF THE BALER AN D PUSHED THE CLAMP BUTTON, ALLOWING THE RAM TO PROCEED INTO THE BALER, CATCHING THE INJURED EMPLOYEE'S ARM CAUSING A LACERATION THAT REQUIRED STITCHES.

EE WAS OPENING A RAILCAR DOOR INSIDE A BOXCAR, IT SLIPPED & FELL DOWN ON HIS BOOT, BREAKING 2 TOES. THERE WAS SNOW IN THE DOORWAYS & IN FLOOR OF THE CAR WHICH MADE FOR SLIPPERY CONDITIONS. HE SLIPPED WHILE OPENING DOOR & HIS FOOT SLIPPED UNDER THE DOOR WHILE TRYING TO OPEN IT. HE WAS WEARING STEEL TOED BOOTS.

TEE WAS TRYING TO OPEN THE BOTTOM HATCH OF A BULK RAILCAR. HE HAD A BAR IN THE SLOT ON THE CRANK AND WAS PUSHING DOWN ON THE BAR. THE BAR SLIPPED OUT OF THE SLOT, AND HE FELL FORWARD, HITTING HIS FORHEAD ON THE RAILROAD TRACKS. CUT REQUIRED 5 STITCHES.
