During the heavy rain down fall near the end of the shift on 2nd shift, a rock from the rock pile came sliding down the pile and struck the right-side window of loader, breaking out the side window and striking operator on left knee area causing a laceration to knee.

On a hot, humid day, an employee passed out due to heat exhaustion. Fell forward and cut a finger on the platform's deck requiring stitches.

Employee was attempting to straighten a lifting eye for one of the Grizzly Back Stops with a 5' pry bar. EE strained and felt a pop in EE's groin - diagnosed as an inguinal hernia.
After shoveling, felt discomfort in abdomen. Went to the doctor the following Monday (04/30/2018) and was diagnosed with a hernia.
At the start of a Monday shift, the Quarry Haul Truck driver bumped the berm while backing into the load out bench, causing a jostling to the driver's body. When dumping the load, the driver felt a twinge in lower back when the driver twisted. Doctor stated strain, and advised one week of no truck driving.

Employee was attempting to move a piece of plate steel that was setting on steel grating. He was using a pry bar to wedge in grating to move the plate. Instead of the plate moving, the grating shifted off the support beam, allowing the grating and employee to fall to the ground below. Sprain wrist and contusion.

Employee was assisting in moving a barge. He was preparing to attach the pull chain to the barge cleat. The cable sheave broke allowing the cable to fall. This jerked his arm causing a fracture to his left forearm.

THE EE WAS HELPING ANOTHER EMPLOYEE MOVE A PIECE OF METAL GRATING FROM A TABLE DOWN TO THE FLOOR. AS THE PAIR BENT DOWN TO PLACE THE GRATING DOWN TO THE FLOOR, ONE EMPLOYEE LET GO OF HIS END OF THE GRATING. THE INJURED EE CONTINUED TO HOLD ON TO HIS END. WHEN THE METAL HIT THE FLOOR THE GRATING CUT THE PALM OF THE EE'S HAND.
EE states he was moving barrels and cables on job site and cut the tip of his finger. Went in for a tetanus shot.

THE REASON THE BRACE TOPPLED OVER TO ONE EVEN THOUGH THE CENTER WAS CORRECT, WAS DUE TO WATER INSIDE THAT COULDN'T BE SEEN OR
