Initial incident occurred 12/31/13, however employee didn't report incident until after Form 7000-2 was submitted on 1/10/14. Due to delay in reporting incident, reporting to MSHA was overlooked, but was reported to Worker's Compensation. An amended 7000-2 will be submitted.
