EE WAS BITTEN BY A COPPERHEAD SNAKE WHILE PICKING UP A SCREW THAT HE HAD DROPPED. SNAKE WAS LYING UNDER RUBBER MAT THAT EE WAS STANDING ON. EE RECEIVED MEDICAL TREATMENT ON 9/25/96. EE WAS BI TTEN ON RIGHT MIDDLE FINGER.
EE WAS CUTTING OFF OLD BUSHING FROM PUSH ARM TRUNION. SLAG BLEW INTO AIR AND A PIECE OF MOLTEN METAL LANDED IN LEFT EAR. ACCIDENT OCCURRED ON 1-26-96. NO LOSS TIME FROM WORK UNTIL SURGERY ON 4 -2-96.

EMPLOYEE WAS TAKING THE MASTER PAD BOLTS OUT OF THE TRACK ON A D11 DOZER USING A1" DRIVE IMPACT WRENCH. THE PIN THAT HOLDS THE SOCKET ON BROKE AND SLIPPED OUT CUTTING HIS LEFT HAND BETWEEN THU MB & INDEX FINGER. HIS LEFT HAND WAS SUPORTING THE WRENCH.

IT WAS DARK AND RAINING AND THE DRAG CHAINS ON THE BUCKET GOT HUNG TOGETHER. THE OPERATOR AND OILER WENT DOWN TO FIX THE SITUATION AND WHILE WALKING, THE OPERATOR SLIPPED IN THE MUD TWISTING A ND STRAINING LOWER BACK TRYING TO BREAK HIS FALL. (HE WAS NOT CARRYING ANYTHING).
WHILE INSTALLING CATWALK AT SCALEHOUSE, CRIBBING WAS USED TO SET THE CATWALK PRIOR TO WELDING CRIBBING SLIPPED AND SHARP EDGE OF ANGLE CUT TIP OF RIGHT INDEX FINGER.
