EE WAS GIVING INSTRUCTION TO EE STARTING ON POT SIDE OF DRAG BENCH HE WAS CROSSING OVER TO SPOIL SIDE TO WALK BEHIND MACHINE WHEN THE HOIST BRAKE FAILED WHICH WAS HOLDING A 12CU BUCKET SUSPEND ED IN AIR THE DRAG ROPE WHEICH 1 21/4 IN DIA ;HIT EE ON HIS HARD KNOCKING HIM OFF BALANCE. THEN HITTING HIS LEG WALK WITH THE FORCE AND POSITION THAT EE HE WAS DISLODACLE AND FRACTURE ANKLE.
