Big Creek Sand and Gravel, Inc. · Fall onto or against objects

This incident initially believed to be a first aid. Worker reached up to open door on wash plant, lost his footing and fell with a hammer in his hand. The hammer bent his right ring finger back. X-ray showed broken finger. No lost time to date and surgery to be scheduled this week will be a day surgery - approximately 1/2 day and then employee will be able to return to work.

HE REACHED OUT AT THE END OF FEED-ER AT THE TOP AND THE PLASTIC WHICH EXTENDED OUT ABOUT 15" BUCKLED DOWN AND DUMPED HIM TO THE GROUND FALLING APPROX. 7'. HE HIT THE GROUND WITH HIS FACE WHICH CAUSED THE INJURIES THAT HE SUS- TAINED. WAS THEN TAKEN TO E.R.
