This incident initially believed to be a first aid. Worker reached up to open door on wash plant, lost his footing and fell with a hammer in his hand. The hammer bent his right ring finger back. X-ray showed broken finger. No lost time to date and surgery to be scheduled this week will be a day surgery - approximately 1/2 day and then employee will be able to return to work.

HE HIT THE GROUND WITH HIS FACE WHICH CAUSED THE INJURIES THAT HE SUS- TAINED. WAS THEN TAKEN TO E.R.

HE WAS MOVING A VEHCILE AWAY FROM PAMT LOCATION WHEN HIS FINGER GOT CAUGHT UNDER THE DOOR HANDLE. HE PULLED HIS FINGER OUT AND AS HE DID IT GOT CUT ON THE INSODE FROM SIDE TO SIDE FROM THE FI RST KNUCKLE TO THE END OF THE FINGER.
EMPL WORKING ON CRUSHER AND AS HE WAS GETTING OFF OF IT HIS FOOT SLIPPED FROM CONVEYOR BELT,CAUSING INJURY TO GROIN AND RIGHT LEG.THIS HURT AN UNDESCENDED TESTICLE,CAUSING IT TO BULGE OUT LIDE A BALLOON WHICH CALLED FOR SURGERY.
