Moving a manbasket, unit hit a bump, resulting in loss of balance and causing him to fall in basket. He was using a safety belt at the time.

A 9" x 7" x 77"long cribbing block was standing on end, ee went to remove block when it tipped over pulling him down with it. His thumb got smashed between the block and concrete floor also with his body weight.
