EE STATES THAT HE WAS IN THE PROCESS OF LIFTING A 50 LB BAG OF MICA FROM THE SEALES TO A PALLET WHEN HE FELT A PULL IN HIS LEFT SHOULDER. HE HAS HIS LEFT SHOULDER IN A SLING FOR HOWEVER IT TAK ES FOR THE HEALING PROCESS. EE SAID NOTHING TO HIS SUPERVISOR OR TO ME UNTIL THE 14TH OF MAY, 2001. HE WORKED THE TWO WEEKS AFTER HIS INCIDENT.

WHILE SCREW CON5EYOR WAS RUNNING THE EMPLOYEE NOTICED A PIECE OF ROOT WRAPPED AROUND SCREW CONVEYOR.INSTEAD OF TURNING OFF THE SCREW CONVETOR,THE EMPLOYEE LEFT THE SCREW CONVEYOR RUNNING,REMO VED THE PROTECT3VE COVER,AND ATTEMPTED TO REMOVED THE PIECE OF ROOT.AT THIS TIME HIS WEDDING RING AND FINGER WERE CAUGHT IN THE SCREW CONVEYOR.ALL OF THE SKIN WAS TORN FROM HIS MIDDLE FINGER.
