INJURED WAS HELPING TO DISAMBLE AHYDRAULIC HOIS TWHICH HA DBEEN REMOVED FROM TRUCK WHEN A CHAIN HOOK SLIPPED OUT OF RING ALLOWING THE CYLINDER TO FALL OVER ON HIS FOOT. THE CYLINDER WAS ON G ROUND AND FELL OVER.

I.NAME REPLACED THE SEDIMENT BULB ON STARTING ENGINE OF SHOVEL. SHOVEL OPERATOR THOUGHT I.NAME WAS CLEAR, AND PUSHED STARTED BUTTON, BUT THE ENGINE DID NOT ENGAGE. I.NAME HAD HIS HAND ON BELT, AND IT DRAWED HIS HAND INTO PULLEY. IT CUT HIS LEFT HAND AND LITTLE FINGER AT KNUCKLE, REQUIRING SOME SURGERY.

CONVEYOR BELT WAS OVERLOADED WITH ROCK, AND THE BREAKER ON MOTOR HEATED UP AND STOPPED. THE ROCK WAS THEN CLEANED OFF OF BELT, EXCEPT FOR FEW FEET AND OPERATOR THOUGHT THAT IT WOULD RUN. IT RA N VERY SLOW, SO HE REACHED TO PULL ON V BELT, AND AT SAME TIME THE BREAKER THREW, AND SENT CONVEYOR BELT IN REVERSE, AND HIS FINGER WAS CAUGHT BETWEEN V BELT & PULLEY. IT PULLED OFF HIS FINGE
