On 6/8/15 the employee was taken to the Company clinic, diagnosed with a right index finger laceration and fracture and was placed on restricted duty which was accommodated. The employee pinched his finger between a pin and an auto leveler guard on a loader during a repair.

On 3/28/15 the employee requested medical treatment and was placed on restricted duty which is being accommodated. On 3/23/15, the employee while installing a 35 lb. screen with a co-worker, alleged low back pain. He was diagnosed with a lumbosacral strain, lumbar radiculopathy and lumbar pain.

On 6/13/14, the employee requested medical treatment, was taken to the company clinic and was prescribed an antibiotic for left eye conjunctivitis. The employee felt something enter his L-eye while using an air impact wrench to tight bolts on a pipe. He was released to full duty.

RAIN CAUSED DIKE ON WATER RECLAMATION POND TO BREAK, ALLOWING APPROXIMATELY THREE FEET OF WATER AND MUD TO FLOW INTO THE PIT AND CRUSHER AREA. THERE WERE NO INJURIES OR EQUIPMENT DAMAGE OTHER THAT MUD CLEANUP.
