Fell from top of staircase of the crusher control house. Did three rolls down the steel staircase scraping both knees, bruising back and neck. No stitches required, employee released by doctor to regular duties as a supervisor.

DURING PLANT MAINTENANCE CABLE USED TO TURN CONE INTO PLACE FAILED THIS WAS NEW CABLE WHICH FAILED AT THE MANUFACUTRERS SOMEONE POINT CABLE HIT WORKER ACROSS BOTH LEGS CAUSING SEVERE BRUISNG A ND MINOR ABRASIONS.
WORKER WAS CUTTING WASH PLANT MOTOR MOUNT FRAME TO REPLACE, MOTOR MOUNT AND MOUNTING FRAME KICKED LEFT, PINNING & SMASHING LEFT HAND LITTLE FINGER BETWEEN WASH PLANT FRAME AND MOUNTING FRAME. SMASHED ONLY MEATY PART OF FINGER CAUSING WATER BLISTER-EMPLOYEE DID NOT SEE DR. UNTIL INFECTION SET IN THRU DELAYING REPORTING OF THIS INCIDENT-TREATMENT ONLY.

EMPLOYEE SLIPPED & GRABBED SHEAVE WITH HIS HAND TO KEEP FROM FALLING MOMENTUM OF ROLLS CARRIED HIS FINGERS INTO PINCH POINT, AMPUTATION THE TIP OF MIDDLE FINGER.
