Picked up a piece of metal to put in the metal bin. Smashed finger against the end of the metal bin. Received a laceration to the first digit of the left hand.

Removing the wheels from the screen trailer with the 1" impact gun. The air discharge from the gun blew the materials in the rim out and something made it past the safety glasses and into the left eye. Foreign body had to be removed from the left eye by a medical professional. No permanent damage or time loss due to this incident.

Driving haul truck up to the load out hopper, stopped the truck and reached out the window to start the conveyor. When the start/stop pendant was contacted, employee received an electrical shock.

Employee was lifting a set of troughing rollers when he discovered a bees nest on the rollers. He attempted to throw the nest away from himself and in the process of throwing felt a sharp pain in his back on the right side. After this incident, the pain was constant.
