Cutting a piece of conveyor belt with a razor knife. The knife slipped and he stabbed his left hand. Employee received 4 stitches to close the wound.

WORKER WAS USING SHOVEL TO CLEAN MUD AROUND SNULHER, SHOVEL GOT CAUGHT IN PULLEY. IN AN ATTEMPT TO PULL SHOVEL OUT WORKER STRAINED HIS SHOULDER. DID NOT REPORT INJURY TO HIS FOREMAN UNTIL 7 DA YS LATER, DIDN'T SEE DR. UNTIL 5-2-94.
