Employee was exiting Bobcat skid steer loader. IW foot slipped off of snow covered tire while exiting. Employee twisted his right knee as he fell to the ground. IW was released to return to light duty work.

Employee was coming down the ladder on a 988 Cat loader. He had fueled the machine up and had the nozzle in his hand. He had three points of contact & he thought he was on the bottom step and realized he wsn't. He missed the bottom step and hurt his right knee. He said it didn't really hurt at the time but the next day it was swollen up.

EE was trying to unplug the chute on the jaw crusher when a plate fell down in the chute and he was using a prybar to try to pry the plate loose when his finger got caught between the bar and the chute. This pinched his middle finger on his left hand. His finger is fractured.

EMPLOYEE WAS CRUSHER OPERATOR AND STOPPED OPERATION TO REPAIR SKIRTING ON PRIMARY CONVEYOR. AS HE WAS ATTEMPTING TO REPAIR SKIRTING HE BUMPED HIS RIGHT KNEE AGAINST AN ANGLE IRON BRACE INJURI NG HIS RIGHT KNEE CAP.
EMPLOYEES WERE TIGHTENING TOP SHELL TO BOTTOM SHELL BOLTS ON ALLIS CHALMERS GYRATORY CRUSHER. EMP. WERE TIGHTENING BOLTS WITH 2 1/4" BOX WRENCH WITH 8' PIPE EXTENSION WHEN APPARENTLY BOLT TURN ED CAUSING BOTH MEN TO LOSE THEIR BALANCE. INJURED FELL FACE FORWARD AND IN DOING SO, HIS LEG BECAME ENTRAPPED CAUSING A BAD SPRAIN IN HIS LEFT CALF AREA.
