Small cut on left hand. Slipped with razor knife while cutting super sack product bag and poked tip of knife in left hand. Minor injury - first aid cleaned and band aid applied. No impairment. After shift employee sent for tetanus shot.

Gold Hill Reclamation and Mining · Fall to the walkway or working surface

We had snow/icy conditions. Preventive measures had been taken in the form of ice melt and sand prior to the injury. Employee slipped and feet came out from under her, falling backwards, hit her head. She walked inside and co-worker asked her what had happened- went for help. Then she was taken by private vehicle to medical facility - diagnosed concussion.

Gold Hill Reclamation and Mining · Struck against stationary object

Worker was pulling a cart across a ramp covering a threshold plate. In the process front legs broke off causing part to fall forward onto worker and edge of cart scraped against his face. Worker then reported incident to supervisor and was then treated for minor lacerations.
