A periodic chest x-ray screening was read by two "B" Readers. The x-ray was characterized as consistent with pnemoconosis with readings of 1/1 and 1/2 . Damage or impairment cannot be determine soley by reading the chest x-ray film.
EE fractured left heel after losing his balance on the upper deck of the Omni cone ladder at the feed box and falling 4 ft. to deck below, weight landing on heel.

I WAS USING CHANNEL LOCKS TO TIGHTEN TIE WRAPS BECAUSE IT WAS RAINING & I COULD NOT PULL THE TIE WRAPS WITH MY HAND. THE CHANNEL LOCKS SLIPPED & FORCED MY THUMB INTO THE CONCRETE.
EE WAS REMOVING LODGED MATERIAL FROM THE DISCHARGE END OF THE PRIMARY CRUSHER. WHILE THE EE WAS ATTEMPTING TO MOVE A SLAB OF MATERIAL NEARBY MATERIAL SHIFTED ENOUGH TO PINCH HIS FINGER TIP.
