The employee was connecting a water hose to the drill to fill with water. Once connected, EE turned back and tripped over the hose that was laying on the ground causing EE to fall and dislocate left middle and index fingers.

An employee was installing pipe supports for a grizzly. The supports consisted of 8" schedule 160 steel 52" long. As employee was installing a support, they pinched their hand between the wall of a the crusher and pipe support causing a laceration require 12 sutures to close the wound.

Trying to lift hood on Mack truck and it caught and employee pulled second time and something popped in shoulder. R. shoulder/cervical strain plan - alternate ice & heat to area, take Naproxen (he already has a prescription) Robavin 750 mg & Ultram 50 mg. Follow up in one week - use sling provided - no use of R. arm.

EE was opening another EE's shoot for dust release, the other EE backed up too far and hit the clam gate on bottom of hopper & impact of clam knocked lever out of EE's hand and knocked him against beam to the ground, causing laceration to EE's left arm.

The employee was greasing an R-35 Euclid haul truck with power greaser. Attempted to grease a fitting on the truck when the grease line busted on end of greaser, causing the grease to enter his left hand pinky finger.

Employee was pounding anvils out of anvil ring with a sledge hammer. Another employee was standing about 25 feet away. While pounding the anvil a piece of metal from sledge hammer, head came off and hit the second employee in the upper left thigh.

EE WAS STANDING ON THE GROUND PULLING ON A 14" PIPE WRENCH WITH A 24" PIPE HANDLE. WRENCH SLIPPED FROM DOUBLE NUTS ON A STUD AND THE EE FELL ON HIS REAR END ON THE GROUND WITH HIS RIGHT FOOT U NDER HIM.

EMPLOYEE WAS DISMOUNTING THE TRAM CAB OF INGERSOLL T-4 DRILL; HE SLIPPED ON THE FOOTSTEP AND STRAINED DIGITS OF THE RIGHT HAND THAT WAS HOLDING TO THE GRID IRON.

EMPLOYEE WAS MOVING A 55 GAL OF OIL WHEN HE STOOD THE BARREL UP HE FELT PAIN IN THE GROIN.

EE DROPPED A BAR USED TO HOLD OPEN AN INSPECTION DOOR OF #2 HAMMER MILL AND MASHED THE 3RD DIGIT OF HIS LEFT HAD. HE SUSTAINED A CONTUSION UNDER THE FINGERNAIL. EE WAS TAKEN TO DR. WHO DRAINED THE BLOOD FROM UNDER THE FINGERNAIL.

EMPLOYEE WAS PINNED BETWEEN A PICK-UP TRUCK AND WRECKER/HOIST TRUCK WHEN THE LATTER WAS STARTED. THE HOIST TRUCK GEAR WAS ENGAGED AND IT MOVED BACKWARDS AT A ANGLE PINNING THE EMPLOYEE CLOSE TO THE IMPACT POINT. INJURY SUSTAINED WAS A FRACTURE TO THE LEFT PELVIS.

A CUSTOMER SUFFERED A HEART ATTACH AND DIED AT THE MINE FOLLOWIN GMANUAL LOADIN GOF HIS PICKUP TRUCK WITH CRUSHED STONE.

EMPLOYEE WAS OPERATING A STOCKPILE HAUL TRUCK WHEN IT OVER TURNED IN A ROADWAY. THE EMPLOYEE APPARENTLY ROUNDED A CORNER EXCESSIVE SPEED.

EMPLOYEE WAS ST9UCK BY A PIECE OF PIPE WHILE DISMANTLING A HOPPER.

EMPLOYEE S HAND3WAS STRUCK WITH A CABLE SLING WHILE DISMANTLING A HOPPER.
