EMPLOYEE WAS GOING DOWN LADDER WHICH WAS PLACED TEMPORAIRLY IN ELEVATOR PIT. WHILE HE WAS ON THE LADDER THE LADDER SLIPPED CAUSING EMPLOYEE TO LOSE HIS BALANCE. HE FELL APPROX. 5\ LANDING ON H IS RIGHT FOOT. HE FELL ON PILE OF GRANULSA AND TWISTED HIS RIGHT KNEE.

EMPLOYEE WAS TIGHTENING BOLT ON THE STAY-NUT FOR ROLLS TAKE-UP ADJUSTMENT. HE WAS USING A BREAKER BAR WITH A SOCKET AND AN EXTENSION PIPE ON THE END. THE RELEASE PIN HOLDING THE SOCKET ONTO BA R WAS JAMMED SO WHEN FORCE WAS APPLIED BREAKER BAR SLIPPED OUT OF SOCKET RESULTING IN MAN\S FINGER BEING COMPRESSED BETWEEN PIPE AND GUARD ON FRONT OF LIFT-TRUCK ON WHICH HIS WORKING PLATFORM
