ROOF BOLTER OPERATOR, EE, WAS INSTALLING THE 4TH BOLT IN THE 1ST ROW OF BOLTS IN THE NO. 9 HEADING WHEN A KETTLE BOTTOM FELL STRIKING HIM ON THE LEFT SIDE NEAR THE RIB CAGE.
EE WAS STANDING IN A WET AREA. HE TOUCHED THE LIGHT ON THE BOLTER AND RECEIVED AN ELECTRIC SHOCK.
WAS LIFTING A PANEL COVER APPROX. 75-80 LIBS. IN 30 INCH COAL HEIGHT. TWISTED BACK AND FELT PAIN WHEN LIFTING LIP UP ON MINER.

CRAWL UP TO MINER TO RAISE HEAD FOR MECHANIC TO WORK ON RIPPER CHAIN. TURNED TO CRAWL BACK FELT SOMETHING PULL IN BACK OF RIGHT KNEE.
OPENED A DOOR ON A BUILDING AND HURT HIS BACK.
