The employee was installing a new trip cable on the Manitowoc 4600 dragline. The cable end came out of the socket and struck EE in the right side of the face.

Brookside Materials LLC · Struck by falling object

The plant was shut down and equipment locked out. Hazmag impact crusher designated as cr1 to do maintenance and the door was opened too far and lid came down and the guard attached to the lid struck the employee in the arm and face. Resulting in fractured cheek bone.
