Technician was troubleshooting an overheat issue on a 345 Excavator. The system was still hot and while troubleshooting the technician attempted to remove the radiator cap. Upon a 1/4 turn twist of the cap the pressure from the cooling package shot the cap off and sprayed hot coolant on the technician.

A pry bar was used to loosen material on feeder and sides in a chiseling motion and employee's hand slipped being cut by the handle of the pry bar.

Employee was in the process changing the final drive oil on Cat 769C haul truck. He was using a breaker bar to loosen drain plug. Pushing down on breaker bar, he said plug came loose causing a snapping sensation in his left shoulder.

EE had just finished a project in the shop. After finishing he started to load pipe onto the service truck. When the drag end fell off the work bench & jarred his left arm.

While operating a Euclid R-35 end dump EE engaged the retarder & the engine died. This happened while going down ramp into pit & the EE lost control & struck rock wall. The EE said he did not try to apply the brake & because of the model he had no emergency steering. The EE got 3 cuts on his head & 1 cut on his arm & because he was wearing his seat belt his injuries were not worse.
While trying to jump start a vehicle with jumper cables an error occurred to supply ignition. The vehicle being started had 2 missing battery caps and it was assumed that they caused the battery to blow up. Fluid sprayed the employees face with no real injury to speak of and the employee was taken to the ER as a precaution.
AN EMPLOYEE USING POOR JUDGEMENT PLACED HANDS ON THE TRUCK BED WHILE IT WAS BEING LOWERED TO THE TRAVEL POSITION. BOTH HANDS WERE PINCHED IN BETWEEN THE TAILGATE & TRUCK BED. MIDDLE FINGERS ON BOTH HANDS WERE BROKE WITH TENDON DAMAGE TO THE OTHER 4 FINGERS.

EMPLOYEES WERE CHANGING ROTOR ON 130/150 UNIVERSAL CRUSHER AND WHEN THEY LIFTED UP THE ROTOR OUT OF CRUSHER EE WAS GOING TO ATTEMPT TO STABILIZEIT AND IT SWUNG BACK INTO HIM AND PUSHED HIM UP AGAINST THE DOOR AND MADE HIM HIT THE BACK OF HIS CALF MUSCLE ON THE INSPECTION DOOR. IT MADE A BRUISE ON HIS CALF AND I TOLD HIM WE SHOULD GET AN X-RAY.
