Employee was repairing a caterpillar 992C when he struck his head on the machine. Laceration required (5) stitches.

Employee was standing on the skid runner portion of a portable water pump, slipped and struck his leg on suction hose screen. He sustained a bone bruise on anterior edge of tibia of left leg.

Drilling an overburden hole, driller helper was dismounting drill when a chain guide broke off, falling approximately 28 feet. Guide struck back of employee's hard hat, exposed section of lower head and then rolled off his neck causing a minor scrape/abrasion.
