While maintenance on the stabilizer bracket for 6A Head pulley gearbox at the dry plant miner #1 removed the bolt off the stabilizer bracket, once the bolt was removed the gear box motor abruptly turned ultimately stopping at the structure I-Beam below. Miner #1 right hand was stuck in between the gear box motor and the I-Beam

While replacing the wiper on DC 6A, Miner #1 cut 95% of the shaft and made decision to kick the shaft to break it. Miner #2 outside of the chute/guard section was holding the shaft to stop it from falling, when miner #1 kicked the wiper shaft it caused the shaft to abruptly move consequently pinching miner #2 Right thump between the guard/chute and the wiper shaft.

While removing the tail section door from belt elevator 6, the employee working the manual chain hoist placed EE's hand approx. 1 ft below the chain on the support structure of the elevator. The door shifted causing temporary slack in the chain which caused the chain to shift down and ultimately pinching the employees left thumb between the chain hoist and the elevator structure.

At approx. 8:45 PM on 06/07/2019, employee suffered a left radial styloid facture due to a fall from a conveyor belt.
