97 Mining Holding Company, LLC · Struck by falling object

Helping mechanics install a hard bar in S11R. Had bar in place with booms of service truck on each side with a strap for rigging. Told to use a wrench to hold shims in place while they finished lining up hole for pin. Could not set with wrench, decided to use their fingers as a strap broke causing injury.

Employee was helping mechanics install a hard bar on D11R. the mechanics had it strapped on each with nylon slings. Employee was given a wrench to hold pads in place, they elected to use their fingers not the wrench as they were putting the bar into place one of the straps tore. Causing employee to loose 3 fingers.

Operator was removing material from the Borden shot area. He pushed material off the highwall side of work area and traveled over the highwall coming to a rest on the Sewickley bench below. Emergency rescue personnel extracted the operator from the dozer and took for medical evaluation as a precautionary measure.

Injured employee mistepped and fell while dismounting a grader after turning on the equipment master switch. He fractured his left wrist in attempt to brace himself while falling.

Injured employee was struck by a hoist cylinder when it unexpectedly broke loose from lifting devices, as it was being removed from a 777D rock truck. The 630 lb. cylinder struck his left leg as he was kneeling in close proximity to the work area fracturing the femur and fibula in his left leg.

A RISH EQUIPMENT MECHANIC PULLED HIS SERVICE TRUCK INTO THE PIT NEXT TO A HITACHI SHOVEL. AFTER MECHANIC WORK WAS COMPLETED ON THE SHOVEL, THE MECHANIC GOT INTO HIS TRUCK TO LEAVE THE PROPERTY . THE SHOVEL OPERATOR STARTED THE SHOVEL AND SWUNG AROUND BEFORE THE MECHANIC WAS CLEAR. THE SHOVEL'S COUNTERWEIGHT STRUCK THE CAB OF SERVICE TRUCK INJURING THE MECHANIC.
