In repairing screener, EE released the open/close lever and in doing so did not anticipate the weight of the equipment. The equipment began to close rapidly and caught EE's finger between the screener and channel iron.

Frozen material jammed auger. Pellet mill operator attempted to free jam by using a metal bar. Bar slipped off of leverage point, pulley recoiled back and pulled hands into belt. Incident resulted in pellet operator losing two fingers on left hand.

Another EE was driving the forklift. IE and other EE were unloading a container filled with 20 2000lb sacks with no skids under. Other EE had lifted the straps for 1 of the sacks about 1/4 of the way in the truck. IE pinched IE's finger between the sack and the tip of the fork lift. was sent to the er.

Walking down steps and caught thumb on an upright and bent EE's thumb backwards
Was filling fuel truck and was walking on a narrow walkway and slipped ee grabbed the railing and fell against fuel truck bruising ribs

while checking the dust collector operator lifted the lid up and removed the clogged dust popped lid up and slid down and cut the operators finger.
entrance gate got wedged between truck and trailer while it was stuck as the truck pulled away the gate was freed and sling shot backwards and hit the employee in the head ( was wearing a hardhat ) as he fell down he broke his right leg.
While shrink wrapping bags with heat gun bag caught on fire he attempted out of impulse to swat the small flame with left hand was wearing latex gloves instead of work gloves. (forgot them at home)
Shoveling material into pay loader bucket, while swinging the shovel over to dump it, he hit the edge on bucket slicing his finger

Working on street sweeper and slipped on oil that was on the motor
Employee was lifting and stacking 60 pound bags. Claimed he hurt shoulder/back.
Employee complained of numbness in left hand and arm while driving forklift. Employee claims this has been ongoing for two weeks. Employee was sent for treatment for left 1st extensor compartment tendonitis and placed on restricted duty.
Employee was ducking under the dryer to look at gear when a bolt sticking out of the dryer knocked off his hard hat. When he reacted to catch it, another bolt struck him on the top of his head.
Employee stepped down off of the yard lift and onto a piece of rock and rolled his right ankle.
Employee was reaching over pellet mixer and bumped bottom rib on left side.
Employee was lifting 60 pound bags of Carb-O-Cite material. He felt sharp pains in his right elbow and couldn't lift any longer.

Employee was greasing equipment when tarp line caught his boot and he fell injuring his right arm.
Employee attempted to lift latch on tanker truck. Latch was rusted and partially frozen shut. Employee felt a pull in groin and left arm near elbow joint.
While loading tanker, pulley on chute broke causing chute to fall on employee's left foot.
Stacking bagged material into a box bottom. The box bottom shifted on the pallet. He picked up the end of the box bottom to move it back on to the pallet. Did not notice any pain until he got home. Reported incident to supervisor on 07/23/13.
Employee was going through the plant doing the morning preventative maintenance checklist. He slipped and fell on the catwalk cutting his left knee and shin.
Employee was bending over and shrink wrapping a pallet of material. After he finished wrapping the pallet, his back began to tighten.

Working on street sweeper changing the hydraulic valve. Employee was wearing safety glasses. Debris in his left eye.
EE was on top of a railcar. Lifted the mat so they can move the car.

EE APPARENTLY WANTED TO CHECK THE BELTS ON THE FAN. HE APPARENTLY REACHED IN BEHIND THE BELT GUARD WHICH WAS IN PLACE, TO FEEL IF THEY WERE LOOSE. WHILE DOING THIS HIS GLOVE GOT CAUGHT BETWEEN THE BELT AND THE SHIV.
