Employee was changing out the striker bar on the drill. When EE loosened 1 of 4 bolts with a wrench, EE felt a pop in the middle of EE's back.

(Separate Sheet)Injured was out of truck after being loaded with rock. He cleaned off tailgate and fell about 4'to the ground injuring his ribs and right shoulder. Driver had passed the Notice to stay in truck sign at least 2 times earlier in the day. Instructions to all personnel if they witness any non-employee out of their vehicle instruct them to get in and stay in vehicle.

Working on crusher with trackhoe, got between crusher and trackhoe attachment, and hoe operator moved the control stick by bumping it. That caused the hammer attachment to fall and strike the employee.
