U.S. Silica Company, LLC · Over-exertion in lifting objects

Over the road driver was removing a tow rope from the mounted bracket on a loader. Employee began lifting the tow rope overhead when they felt a pop in their left shoulder, followed by immediate discomfort in that shoulder. Employee was evaluated and diagnosed with a strain to their left shoulder.

Employee states being towed through the mud at mine site (after getting stuck in mud) created jerking motions which resulted in shoulder, rib, neck, and back pain. Pain could also possibly be due to repetitive motion involved in assigned duties of driving. Driver first reported to Employer on 2/17/14.

Mine road had very wet, muddy, and slick due to rain. Employee was walking on the wet ground. Employee then slipped and fell on right shoulder and felt great pain in shoulder. Employee is part-time and only works once in a while. We did not need him to fill in after the injury.

Driver was loading reject at plant when pin came out of handle on tarp covering reject material and spun around catching his forearm. Bruise on R forearm. Received tetnaus injection and injection for swelling. Discharged back to full duty. Employee normal days off followed injury so no loss work days.

Employee was changing an oil filter on a Clark 290. To reach the canister the EE was standing on a 6'ladder (3rd step). The ladder was leaning against the equipment & slipped causing the EE to fall. This caused back pain, this injury was not reported until 12/12/06.
