While climbing a 4' step ladder, employee strained his left knee. No unusual behavior was noted, just the knee failed

Employee states being towed through the mud at mine site (after getting stuck in mud) created jerking motions which resulted in shoulder, rib, neck, and back pain. Pain could also possibly be due to repetitive motion involved in assigned duties of driving. Driver first reported to Employer on 2/17/14.

Mine road had very wet, muddy, and slick due to rain. Employee was walking on the wet ground. Employee then slipped and fell on right shoulder and felt great pain in shoulder. Employee is part-time and only works once in a while. We did not need him to fill in after the injury.

Driver was loading reject at plant when pin came out of handle on tarp covering reject material and spun around catching his forearm. Bruise on R forearm. Received tetnaus injection and injection for swelling. Discharged back to full duty. Employee normal days off followed injury so no loss work days.

Truck driver was exiting truck and fell off of step. Factured lower left leg.

Employee was changing an oil filter on a Clark 290. To reach the canister the EE was standing on a 6'ladder (3rd step). The ladder was leaning against the equipment & slipped causing the EE to fall. This caused back pain, this injury was not reported until 12/12/06.

Lifting a five gallon bucket of oil. up into the back of the service truck lost footing. causing him to slip and twisted his lower back.

While refueling the level of fuel spilled out over the tank. Climbed down and stepped across to the top of tool box and lost footing, cause of fuel on bottom of boots and fell to ground landing on feet.

WORKER CLAIMED THAT RIDING AROUND IN A 631 SCRAPER WITH A LOOSE SEAT CAUSED INJURY TO HIS BACK.

WHILE STEPPING OUT OF A D9 DOZER, WORKER STEPPED ON A ROCK & TWISTED HIS LEFT ANKLE. NOTIFIED EMPLOYER ON 06/14/2002. ONE FOLLOW-UP VISIT, PRESCRIPTION. SHIFT 4 ON, 4 OFF.

WHILE CHANGING A TIRE ON A GRADER, WORKING SMASHED HIS OWN HAND WITH A HAMMER.

WORKER WAS ATTEMPTING TO LIFT A D-9 BATTERY IN ORDER TO PLACE IT IN THE BACK OF A SERVICE TRUCK. THE COMBINATION OF LIFTING AND TURNING CAUSED A LOWER BACK SPRAIN.

WHILE BENDING OVER TO RETRIEVE ORE SAMPLES FROM THE SAMPLE BARREL, WORKER STRAINED HIS LOWER BACK.

WHILE STEPPING OUT OF THE CAB OF HIS 631E SCRAPER, WORKER SLAMMED HIS LEFT THUMB IN THE DOOR OF THE CAB.

CABLE COME-A-LONG BROKE AND EMPLOYEE FELL ON APRON OF SCRAPER.

DRIVING SCRAPER-DROVE FRONT END OF TRACTOR OFF EDGE OF STOCK-PILE UNHOOKED SEAT BELT JUMPED/FELL OFF.
