HE SLIPPED ON LADDER FRAME TWISTING KNEE. ON 9-9-03 TWISTED IT AGAIN WHEN STEP STOOP TURNED OVER IN EMP. ROOM.
HE TRIPPED WHILE CARRYING A SCREEN. (HE WAS CARRYING A SCREEN AND I HAD TOLD HIM TO BE VERY CAREFUL AND WATCH WHERE HE WAS WALKING AND NOT TRY TO CARRY THE SCREEN BY HIMSELF WHICH HE DID WHEN HE FELL.).

BOSS TOLD EMPLOYEE TO PUT THE HERKONDIZER ON 966 LOADER, BUT FIRST TAKE A CHAIN AND LIFT UNITON A BLOCK SO IT WOULD BE EASIER TO INSTALL. HE DID NOT DO AS INSTRUCTED. THE LIFTING DEVICE HIT EMPLOYEE IN THE ANKLE.
