EE PLACED PERSONAL TOOL BOX ON GATE DRIVE UNIT..ENTERED OFFICE VIA SHOP TO BACK SIDE OF RETRACTING SECURITY GATE. REACHED THROUGH AN OPENING WHERE WIRE WAS MISSING TO PICK UP TOOL BOX. GATE SUDDENLY OPENED PINCHING EE & DRUG HIM BETWEEN GATE AND FENCE BREAKING RIGHT LOWER LEG & INJURING LEFT SHOULDER LACERATION TO RIGHT EYE BROW.

INJURED HAD TORCH CUT A PIECE OF 3/4" PLATE 16' WIDE X30" LONG WEIGHING APPROX 90 LBS AS HE ATTEMPTED TO MOVE THE METAL IT FELL FROM THE TABLE STRIKING HIM ON THE LEFT FOOT BREAKING 2 BONES IN THE TOP OF THE FOOT-NOTE REASON ROLLA FORM BEIN LATE I DID NOT HAVE FORMS SO I ORDERED FORMS FROM MSHA-ROLLA MO.PLEASE EXCUSE.

I.NAME MAKING SEVERAL LIFTING LUGS BY PUNCHING A SERIES OF HOLES IN A 1/2" X 4" BAR.THEN CUT THE BARS TO 8" LENGTH.TO CUT OFF CORNERS USED THE COPING SAW RATHER THAN THE SHEAR CUTTER.THE MOVEA BLE COPING BLADE WEDGED AGAINST STATIONARY BLADE .BAR WAS WEDGED BETWEEN THE BLADES AND METAL FRAGMENTS PUNCTURED I.NAME IN SEVERAL PLACES 3 OR 4.
