As they were prying loose a rock on the rib, they felt pain in their abdominal area and then continued working the remainder of the shift.

AFTER THIS THE EMPLOYEE STARTED HAVING LOWER BACK ISSUES.

The stand-up ditch witch caught a hose on the wall in the tunnel on the back door cover plate on the hydraulic side, which pulled the machine sideways and propped the door like it was running over a rock, and when it did it caught the right thumb on the hand rest, there for partially amputating the right thumb

EE DID NOT LOOK DOWN WHERE HE WAS WALKING AND HE WALKED INTO THE STAIRWAY END OF PIT. THIS CAUSED HIM TO FALL INTO THE PIT. X-RAYS SHOWED HE BROKE THE LITTLE TOE ON HIS RT FOOT.
