1:00 pm #29 Mine hoist went down. After evaluation it was determined to be a skip hung in shaft. MSHA was called and evacuation procedures were in process. Evacuation process completed at 4:45 pm. Maintenance was completed to put shaft back into service on 12/30/17.

On 11-23-09 the main hoist suffered a control module failure at the #29 mine. The hoist failed at 2:34pm and MSHA was notified at 2:37pm. The hoist was back in service at 5:00pm and MSHA was notified at 5:05pm.

#29 mine hoist lost power at 8:53am due to electrical failure. MSHA was notified by phone at 8:58 and MSHA was notified again at 11:20 that hoist was back in service.

EE was attempting to move a boulder. He put his hand on wire rope while pushing the down button on crane control wire rope pulled his finger into the sheeve. Smashing pinky and ring finger on right hand. EE was taken to ER and later transferred to Des Peres Hospital where he had surgery on 11-17-09 making this a lost time accident.

EE was operating a forklift on the surface, EE went to adjust the left fork and it came completey off hitting top of EE's right foot. EE was seen by ER and released to CO. Dr. after x-rays it was revealed a possible fracture/contusion, referred to Ortho Dr. EE released to sedentary work only.

Co-worker approached the injured EE from behind and struck him behind the right knee, causing him to loose his balance and right knee popped. After continuous treatment, the physician gave him restrictions that caused him not to be able to perform his normal job description. This became a lost time incident on 2/20/08.

Employee was dismounting the truck to check a tire when he lost his grip on the handrail and fell about five feet to the ground. Employee was taken to the emergency room where he received 19 sutures to his finger.

Power went out causing hoist to go down at 8:40 A.M. Problem was with electric company (Amerun UE). Hoist was back in service at 11:30 A.M. MSHA was notified of power outage and when power was restored.

Boulder hung in load chute in skip pocket. Boulder was too big to go thru the load out system getting thru grizzly. While trying to move cable, hook slipped off boulder hitting the employee across the nose. Employee was taken to emergency room where he was diagnosed with a crushed bridge and would need surgery. Employee is currently off work.

EE WAS FILLING THE HYDRAULIC TANK ON #93 TRUCK. AS HE PREPARED TO DISMOUNT THE GRAB HANDLE BROKE. THIS CAUSED THE EE TO FALL INTO PILLAR NEAR THE HYDRAULIC SWITCH. EE HURT LEFT SHOULDER, NECK & BACK. ON 5/15/05 EE WAS PUT ON LIMITED DUTY OF NO LOOKING OVERHEAD & NO LIFTING OVER 30 LBS.

AT 1:55 PM, SATURDAY, 11-3-84, #29 MINE LOST ALL POWER. THE FUSES ON LINES COMING INTO TRANSFORMER OVERHEATED AND AT LEAST ONE CAUGHT FIRE. THE FUSE HOLDERS WERE CLEANED, FUSES WERE REPLACED, AND A 2300 BOLT LINE GOING UNDERGROUND WAS REPAIRED. HOISTING WAS INTERRUPED WHILE FUSED WERE REPLACED. MINE CREW WAS EVACUATED BEFORE THE 2300 BOLT LINE WAS REPAIRED.
