WHILE PREPARING CONVEYOR BELT FOR INSTALLATION INTO A CHUTE FOR WEAR AND NOISE RESISTANCE. HE WAS STANDING ON THE PLATFORM AND REACHED UP TO RECEIVE THE PIECE OF CONVEYOR BELT AND THEN STEPPED BACK ONTO THE PLATFORM PUTTING HIS LEFT FOOT ON A PIECE OF WOOD CAUSING HIM TO LOSE HIS BALANCE. HE SLIPPED AND FELL 10 FEET FROM CATWAY WALKWAY.
