Employee felt a pop in left knee when mounting water truck. Employee has had a previous knee injury. Not determined until May 6th that this was a new injury and not a reoccurrence.

The employee was helping the mechanic to replace the idler on the Komatsu PC220 trackhoe. They were using a hammer to hit a chisel into a slot to help the shaft slide into place. The chisel unexpectedly popped out of the slot and came back and hit the employee in EE's open mouth, injuring two of EE's teeth.

Operator was hauling a load of spoil material to the dump area, drove into a hole and right leg contacted the bottom edge of dash resulting in a small laceration just below the knee about 1/2 inch long requiring one stitch. Employee was treated and given a return to work release.

While exiting WA500 loader, employee slipped off and fell to ground below. Landed on left foot and left hand. Employee was standing on top step of ladder and had lunch box in left hand holding handrail. When employee closed the door he reached to grab hand rail with right hand and missed, lost his balance and fell to the ground.

Employee was entering the Chattanooga scale house when her foot slipped off of step and she lost her balance and fell on stairs, breaking her fall with her left arm. Employee's toe caught the end of the step causing her to trip. She has fractured her left arm.

Employee was performing the daily servicing tasks (fueling & greasing) on the Kamatsu PC 750 excavator located in the bottom of the pit. While exiting the machine employee lost his footing and fell approximately 3 feet to the ground.

EE WAS USING A ROD TO PRY A ROCK FROM BEHIND WHEEL ON SCREEN CHUTE, THE BAR SLIPPED AND HIS HAND HIT A PIECE OF ANGLE IRON SUPPORT ON THE CHUTE. LITTLE FINGER OF RIGHT HAND WAS LACERATEC CAUSING HIM TO RECIEVE TWO STITCHES.
