A non-injury roof fall was discovered during the weekly examination on the # 2 Section near spad no. 581. Fall measured approximately 60 feet long x 20 feet wide x 14 feet high. Fall has been timbered off and danger signs posted on all approaches. This mine is in non-producing status.

Employee was lifting cable bolts off of the top of 5ft bolts that were in an insert to load the roof bolter.
Loading top rollers for conveyor belt into scoop insert and felt pain in abdomen. Injured continued to work up until 08/12/2024. Safety Department was notified on 08/19/2024 that injured has a hernia (unrepaired) diagnosis. Injured returned to work on 08/19/2024. Standard time charge of 50 days applies.
Walking down incline slipped in gravel and mud; knee bent backwards (hyper extended); walking from parking lot to bath house
Employee was assisting in unloading supplies when a pallet of block on the fork lift hit the water line causing it to fall, striking the employee on the left ankle and track rail.
Employee was getting out of the middle of the shuttle car and twisted their left knee when they stepped on a rock in the mud.
Employee was watching 8" Poly Pipe as Motor was pulling it up the track entry when the tail of the pipe swung around striking the employee in the left leg.
Employee was inching up to install a rib bolt when the canopy struck a cable bolt popping the nut off causing it to fly off and strike the employee in the upper lip.

Employee was walking down the entry and slipped on a ledge hurting their left knee. This was a file only accident and the employee continued to work until 6-22-2024. This is now a Lost Time.
