CHANGE OF DRIVE SHEAVES WAS BEING PERFORMED ON A FEEDER BELT. THE TORK ARM WAS REMOVED FROM THE GEAR BOX & CABLE PULLER WAS USED TO ROTATE THE GEAR BOX. GEAR BOX WAS ROTATED WITHOUT SECURING THE FREE END WHICH ALLOWED THE GEAR BOX TO FALL. THIS PINNED THE MINERS HAND BETWEEN THE GEAR BOX & FRAME.
