EMPLOYEE WAS OPERATING RUBBER TIRED LOADER. HE REACHED TO OPEN RIGHT SIDE DOOR AND HIT ARM REST LEVER INJURING HIS RIBS. EMPLOYEE DID NOT FEEL IT WAS SERIOUS AND CONTINUED TO WORK THRU 8/13/05. HE WENT TO EMERGENCY ROOM THE NIGHT OF 8/13/05. X-RAYS WERE INCONCLUSIVE.

WAS ENERGIZING LINE TO IMPACT FEEDER BELT WIRE HAD GOTTEN TRIP SWITCH. WHEN SWITCH WAS ENGAGED CAUSED THE WIRE TO SHORT BEHIND LEVER AND BOX CAUSING A FIRE BALL TO FORM. BLOWING LID OPEN AND BURNING FACE AND HAND AREA OF PLANT MGR.
