EE informed foreman he felt a twitch in his leg but it did not hurt when it happened. When asked if he wanted to see a dr. the EE said he would wait,stating? it could just be a pulled muscle?. On 6/12/2008 the EE went to see a surgeon and was diagnosed with a hernia. On 6/24/2008 the injury was determined to be work related. To date the EE has not missed any time from work.

Wedge kicked back from crusher and pushed employees arm into handrail. He noticed some swelling in his arm after the incident so an ice pack was used to reduce swelling. The employee said he felt like he was OK. On 10/25/2007 he became concerned about the extent of the bruising on his arm and requested a doctor's appointment. He was taken to the doctor, x-rays were negative.
Employee was pulling on a wrench when he heard something pop in his side. Employee did not think he needed to see the doctor at the time but on Monday 8/10/2007 he said he was still hurting and wanted to see the doctor.
The employee was rotating a motor sheave, when it came off of the shaft. The sheave fell, landing on his right ring finger, breaking the tip of the finger.

Maintenance crew was preparing to install new manganese in crusher. Pittman was jacked forward to allow the shims to be removed. Employee was pulling shims out when the jack lost pressure and crusher closed while his fingers were between the shims.
The employee was handing a 2 foot section of poly pipe to another employee. The other employee dropped the pipe, and it struck him on the hand resulting in a contusion. He did not go to the doctor until 10/27/05 where he was placed on modified duty.
The employee was standing in a chute replacing bolts that hold the liner in, and slipped and fell approximately 3 feet.
Employee was using torch to cut bolts off of a roller and got slag in his ear. Initially, employee refused medical treatment at the time. On June 21, after experiencing severe pain as a result of foreign particles in right ear, employee decided to seek medical treatment.
Employee was working in the same area with another employee using grinder. Emoployee removed safety glasses to clean them and was struck in eye by small metal shaving by grinder. The piece of metal was removed, but a prescription eye drop was given.

The employee was grinding with an electrical grinder and got a small metal sliver in his eyes. The employee was wearing safety glasses. The employee did not report the injury to his supervisor, he went to get a physical and told the doctor about the sliver. The employee has been terminated following the post injury drug test.
The employee was picking up a grinder with his finger on the trigger and inadvertently engaged the trigger causing the grinder to twist and strike him in the leg.

EE WAS CLEANING OFF SOME STEEL PRIOR TO CUTTING WHEN HE GOT SOME DEBRIS IN HIS LEFT EYE. HE WAS WEARING SAFETY GLASSES AT THE TIME.
EMPLOYEE HIT A HIGH BOTTOM ON THE PIT FLOOR. THIS CAUSED HIM TO INJURE HIS BACK.
EE WAS SITTING ON A STEEL PLATE SECURED BY A CHAIN HOIST. THE HOIST SLIPPED, CAUSING HIM TO FALLDOWN INTO A CHUTE. HE RECEIVED 2 STITCHES IN HISRIGHT KNEE.
EMPLOYEE WAS ATTEMPTING TO LIFT ELECTRICAL CABLE THAT HE HAD JUST UNROLLED. EMPLOYEE IMPROPERLY ATTEMPTED TO LIFT THE CABLE AND CAUSED HIMSELF TO STRAIN HIS BACK.

ENERGETIC SOLUTIONS WAS BLASTING 50' PRODUCTION SHOT ON WEST END OF PIT. SHOT WAS TRIGGERED AT 4:10PM BY JEFF GOOCH. SHOT CONSISTED OF 5" DIA. BORE HOLES ON 12 X 13 PATTERN. FLY ROCK WAS THROW N FROM BLAST HITTING AND CUTTING POWER LINES ANDA FIBRE OPTIC CABLE. ADJOINING PROPERTIES WERE ALSO STRUCK WITH 5" AND SMALLER ROCK. CAUSE IS UNKNOWN, BUT VIDEO TAPE WILL BE REVIEWED.
EMPLOYEE WAS DESCENDING THE PIT LOADER LADDER WHEN HIS FOOT SLIPPED. HE FELL ON THE PIT FLOOR, TWISTING HIS RIGHT KNEE.
EE SAID A BOULDER FELL FROM THE LOADER BUCKET CAUSING THE TRUCK TO BOUNCE.
EE WAS WALKING DOWN A STEEP BANK TO RETRIEVE A SUBMERSIBLE PUMP FROM THE CREEK.
EMPLOYEE WAS HELPING TO REMOVE THE LOCKING NUT FROM THE TERTIORY CRUSHER. WHILE ATTEMPTING TO SWING A RAM INTO THE NUT, EMPLOYEE GOT HIS FINGER BETWEEN THE RAM AND THE NUT AND SMASHED THE MIDD LE FINGER.
EE WAS REMOVING SINK STOPPER USING POCKET KNIFE AS A SCREWDRIVER. BLADE FOLDED AND CUT INDEX FINGER.

WHILE ATTEMPTING TO REMOVE ROD AND REPLACE IT BETWEEN TWO COUPLINGS EE BROUGHT THE TWO COUPLINGS TOGETHER BY HAND CONTROL. HE MISJUDGED DISTANCE AND SMASHED FINGERS OF LEFT HAND.
