Approximately one month ago, employee was operating the grader in the clay pits. As he was getting out of the grader, employee hit his right elbow on the metal ladder. He did not seek treatment at that time. On 07-27-09, the employee reported to the office that the elbow had developed fluid on the joint.

Employee was operating an excavator in the clay pits. He brought the back end of the machine down hard on the ground while sitting on the edge of the seat and felt pain in his lower back.

A 4" WATER PUMP INTAKE APPARENTLY BECAME PLUGGED& AS THE PUMP RAN DRY HEATED THE REMAINING WATERIN THE HOUSING; EMPLOYEE WHEN INSPECTING PUMP NOTICED THE CAP ON THE PUMP HOUSING WAS LOOSE & LEAKING; HE TRIED TO TIGHTEN THE CAP WHILE THE PUMP WAS RUNNING. CAP POPPED OPEN IN HIS HAND, SPLASHING STEAM & HOT WATER OVER HIS LEFT ARM, CHEST AND LEFT SIDE OF HIS FACE.

EE RECEIVED A HAIRLINE FRACT URE TO WRIST AND DR RESTRICTED WORK ACTIVITY FOR 2 WEEKS AT LESS THAN 10 POUNDS. ANTICAPATE FULL WORK RELEASE 4-21-00.
