EE WAS MAKING A SPLICE IN A WATER LINE, A METAL BAND WAS STUCK IN THE MACHINE. AS HE WAS PULLING IT OUT OF THE MACHINE THE BAND CUT HIS FINGER.
VICTIM TRIPPED OVER BUGGY CABLE TURNING ANKLE.

EMPLOYEE WAS SHOVELING ALONG THE CONVEYOR BELT AND STRAINED HIS LOWER BACK.
SHUTTLE CAR WAS STUCK UP AND A SCOOP WITH A WINCH WAS BEING USED TO PULL SHUTTLE CAR AROUND. FLOOR UNDERNEATH SHUTTLE CAR GAVE WAY AND CAR SLID SUDDENLY, STRIKING EMPS LEFT LEG.
HE WAS USING A HAND GRINDER WHEN IT KICKED BACK AND STRUCK HIM ON THE LEFT THIGH AREA CAUSING AN ABRASION. IT LATER BECAME INFECTED CAUSING HIM TO LOSE WORK.
EE WAS CLEANING DUST OUT OF BOLT MACHINE WHEN A PIECE OF LOOSE ROCK FELL STRIKING HIM IN THE BACK.
