Teichert Aggregates · Struck against a moving object

While a 385 Excavator was loading a 745 CAT Haul truck, a rock est. 500-1000 pounds rolled over the truck bed. The passenger side hand rail, a stair tread, and a hose bracket were bent. No damage to the operator cab. Employee refused post accident drug and alcohol screen, left the site without notice prior to end of shift, and later resigned via text msg.

While lifting the forklift blade to adjust the horizontal location, the employee got her right pinky finger smashed between the blade and the carriage. This resulted in a hairline fracture and sutures to close the wound.

While stepping off the back of the excavator, the operator slipped on a rock causing him to fall onto sharp broken rocks. As a result the operator cut his left hand beginning at the wrist.
