Employee was operating a Fletcher Roof Bolter, at which time a piece of loose metal on the roof bolt snagged the employee's glove. When the employee moved the joystick on the roof bolter, the bolt twisted the employee's glove dislocating two fingers.

Employee was referred for MRI. Results were received on 03/16. Results indicate occupational illness in the form of early degenerative disc disease of the lower 5 lumbar levels.

Employee was attempting to wash out rock box at #5 Belt Head. When Employee turned on the fire hose the pressure knocked him down causing injury to his left shoulder area. Employee was transported to Raleigh General and then due to the nature of injury transported to CAMC General Division and was diagnosed with a broken clavicle and admitted for observation.

Employee was treated at MedExpress and was instructed to follow-up with a specialist. The metal was slightly embedded in eye and was removed.

While making a splice on the #1 Overland Belt EE bent over and attempted to pick up a set of chain ratchets and lift them over his head. He stated that he felt a sudden pain in his lower back. After seeking medical attention he was diagnosed with a strain. EE had scheduled a personal day to be off on 06-29-10 before this accident. He returned to work on 06-30-10.

EE was traveling on the roadway to the mine to begin work. As he started up the hill his truck lost traction . He attempted to get out of the truck, slipped on the slick roadway and fell, causing him to slide under his truck, hitting his chest.

EE attempted to turn up a 1000' spool of communication and felt a pain in his back. EE continued to work until 03-08-2010. On 03-09-2010 he notified the Safety Department that due to continued pain he has seen a doctor and was dianosed with a lumbar protruding disc and could not work.

WHILE REPLACING THE HOSE AN UNKNOWN AMOUNT OF (CAP COAL) FELL FROM THE MINE ROOF STRIKING EE ON HIS HARDHAT RESULTING IN SORENESS TO HIS NECK.

UPON STRIKING THE BIT A PIECE OF THE HAMMER HEAD BROKE AND PENETRATED THE SKIN, IMBEDDIN G IN HIS RIGHT THUMB. EE DID NOT SEEK MEDICAL TREATMENT FOR THIS INJURY UNTIL 4-18-02. THE METAL WAS SURGICALLY REMOVED.

THE MINE FLOOR IS SOFT, WHICH CAUSES IT TO BE UNEVEN ONCEEQUIPMENT HAS TRAVELED OVER IT. HE SOUGHT MEDICAL ATTENTION ON 1/28/02, DIAGNOSED WITH CONTUSION
