RIDING IN RUBBER TIRE MANTRIP RAISED BACK UP INTO TOP HURT LOWER BACK AND SHOULDERS.
THE EMPLOYEE WAS USING A RATCHET TO REMOVE A REEL RATCHET KICKED UP AND STRUCK THE EMPLOYEE INTHE MOUTH.
MOVED UP 4 FT TO INSTALL NEXT BOLT, SWUNG HEAD OUT AND SET ROOF JACK. PUT STEEL IN HEAD AND PUT IT AGAINST TOP, STARTED ROTATING STEEL AND A CAP COAL FELL. (5 IN THICK 2 FT WIDE 6 FT LONG).

THE EMPLOYEE WAS STANDING INSIDE A S & S UNITRAC BUCKET. HE WAS PULLING A SMALL ROCKDUSTER INTO THE SCOOP BUCKET WHEN HE FELL ON THE SLIPPERY METAL CAUSING A MINOR FRACTURE TO HIS LEFT ANKLE.
