Employee parked the end-dump truck he was operating and was walking to his personal vehicle. He slipped on the snow and fell on his left wrist in the parking area of a surface mine site.

Employee had dismounted a bull dozer and slipped on some frosty grass falling on his right side.
Employee was assisting in the tear down and loading of D10N dozer onto a lowboy. The ROPS structure was removed and was being placed on the rear deck of the lowboy. As the structure was set down a lifting chain came loose and struck the employee.
EMPLOYEE WAS WALKING TO THE OFF ROAD TRUCK HE WAS GOING TO OPERATE WHEN AN INSIDE TIRE OFF A PARKED TRUCK BLEW-UP CAUSING A LOUD NOISE.

Mechanic was unloading a water pump with a mechanic's truck and hydraulic crane. When the mechanic set the pump down he slipped on the mud and caught his left hand in the lift bar on the pump and smashed his finger.
