EE WAS USING LADDER TO PAINT EQUIPMENT NUMBERS ON PROCESSING EQUIPMENT. THE LADDER WAS NOT SECURED AT THE TOP AND EE FELL APPROXIMATELY 6'. AFTER FALL EE COMPLAINED OF HEADACHE, SORE BACK, AND HAND AND ARM PAIN. WAS TAKEN TO THE EMERGENCY ROOM FOR CHECK-UP.
