when getting a rock off the dolly it slipped out of EE's hand and came down on EE's right little finger. Finger was between the rock and the dolly frame resulting in a broken finger.

using a pry bar to assist the mechanics in changing the coal chain on the miner, the bar slipped and they struck their hand on a piece of metal resulting in a fracture of the right index finger tip. NO WORK MISSED.

when shoveling in the beltline the rib rolled down onto their left leg resulting in bruising. Employee was transported to the hospital for precautions but was released and the doctor said no work for a week to rest their leg.

Employee took the first side of a new cut in the number four entry when the roof started working. EE backed the CM outby the start of the cut about 20' when a rock fell out between the T3 channels striking EE in the back/ankle resulting in broken vertebrae and ankle.

EE was removing the rub rail off the joy miner when it fell and struck EE's right foot just above the steel toe resulting in 2 broken toes. EE chose to continue to work through this and not miss any work. Toes are taped together and EE is back underground. No work missed.

Loading plates onto the bolter EE's shoulder popped. No work was missed until starting today 9/30/24 because EE has been running the MBC and taking it easy. On 9/30/24 EE came to work and said cannot go underground because it does not seem to be helping thus starting time off.

When reaching for a stack of plates on the roof bolter a piece of rock fell on EE's left hand resulting in a dislocation of the middle finger and slight break under the nail bed of the pointer finger. Doctor gave EE a splint and NO work missed at all. underground the next day at full duty.

When setting up to pull belt off EE dropped the belt pulling arm on left foot just behind the steel toe. The doctor's report said nothing broke but told EE to wear a walking boot, ice it and rest it. EE has been sweeping floors and cleaning outside since that date.

Normal working section, EE was cleaning some rock off the miner. A piece of rock EE was taking off the miner flipped off and struck EE's left thumb and caused a laceration which resulted in 6 stitches. NO work missed and NO restrictions from the doctor. Back to work at full duty.

The operator was backing out the first side of the cut when they were struck by a piece of coal rib that fell out behind the curtain striking them in the lower back. This pushed them into the miner where they struck their face. The incident resulted in a fractured L-2, broken nose and 6 sutures above their nose.

When traveling outside as a passenger on a 2 man personnel cart, the driver hit a bump and bottomed out. Employee was holding onto the side of the cart with fingers under the cart. When the cart bottomed out it got the fingers resulting in 5 stitches in little finger and broke the tip of the little finger. No work missed, employee is back 2/4/2022.

When installing a cable bolt into the roof, the head of the bolt and the plate was still on the ground. EE stumbled and went down on left knee and struck the plate just above knee pad resulting in a cut that received 3 stitches.

While the operator was drilling in wet conditions, EE lost suction. At this time EE dropped the drill head to the bottom and placed EE's hand over the chuck to check the suction pressure. When doing so the drill steels fell from the hole striking EE on the top of EE's right hand causing a break.
