Employee stated he was in a boat when he was attempting to lift some heavy pipe and the boat moved causing him to twist and injury his back. He did not report to me until 6/11/07 at which time he raised his leg to climb steps and was unable to move.

Employee struck forehead on pipe flange and cut his head requiring stitches.
ACCIDENT NOT REPORTED UNTIL 7/5/05. WORKING ON DREDGE REBUILD. EE STATED HE SLIPPED AND FELL WHILE TAKING HYDRAULIC PUMP OFF. THERE WERE NO WITNESSES. THIS ACCIDENT WAS ALLEGED AFTER EE WAS WRITTEN UP FOR NOT REPORTING FOR WORK AND TERMINATION WAS BEING CONSIDERED.
TWO EMPLOYEES WERE MANUALLY LIFTING GENERATOR OFF THE DREDGE. AS THEY WERE LIFTING OFF THE STAND, THE LEFT CORNER ON INJURED EMPLOYEE'S SIDE CAUGHT THE CORNER OF THE STAND AND CAUSED THE GENERATOR TO TILT. THE GENERATOR FELL AND SMASHED HIS THUMB BETWEEN THE GENERATOR AND 10 INCH PIPE.
