TWO WORKERS WERE REMOVING LIMESTONE FROM A RECENT SHOT. AFTER LOADING WAS COMPLETED USING A 235 EXCAVATOR, THE HAUL TRUCK DRIVER WALKED TOWARDS THE HOE PARELLEL TO THE TRUCK FROM THE FRONT TO REAR DIRECTLY BESIDE AND BELOW TO ABOUT THE REAR WHEELS OF THE TRUCK WHEN A PIECE OF LIMESTONE APPROX. 20 LB. FELL FROM THE BED STRIKING HIS HARD HAT AND SENDING HIM TO THE GROUND.

TWO WORKERS WERE DOING REPAIR WORK ON THE TOP DECK OF THE TELESMITH SCREEN WHICH REQUIRED SOME WELDING. ONE WAS HELPING HOLD THE METAL TO BE WELDED INTO PLACE WHILE THE OTHER DID THE WELDING. HE APPARENTLY SUFFERED SOME FLASH BURNS WHILE THE METAL WAS BEING TACKED INTO PLACE EVEN THOUGH HE WAS IN THE OPEN & NOT LOOKING DIRECTLY AT THE ARC. HE DIDN'T HAVE DIFFICULTY UNTIL THAT NIGHT
