Operator was making a hard left turn up a ramp when the right front tire caught traction and pulled the cab over on the right side of machine.

RGI was injured while working on an excavator engine hood. The employee had the engine hood in the open position with the hinges clamped into place and was working from a ladder. The employee was working overhead, welding and repairing the hinges and was wearing safety glasses, welding hood, and helmet during this operation and had performed a verbal JSA on this task.

Employee lost balance after breaking an excessively tight fitting loose on a machine falling backward landing on left hand and fracturing left wrist. The employee was working in a tight area between the hydraulic tank and front frame about a foot off the ground.
