Trouble shooting on electrical starter in control van. Employee went to de-energize breaker and the breaker malfunctioned causing small debris to discharge into employee's work area. Debris went inside the top employee's safety glasses. Employee suffered laceration to surface of eye and no impairment is expected.
The employee was trying to adjust a chain on the telescoping conveyor. The cable clamp holding the sliding conveyor in place came apart and the conveyor belt slid down crushing the employee.
The employee was walking across the yard and slipped and fell in the snow. Hitting the back of EE's head.

The employee was on a conveyer belt. Harness was on. The employee failed to follow lockout tagout procedures. The belt was started and the employee jumped from the belt causing a hairline fracture to the right ankle. Employee is currently on light duty.
