Preston Ready Mix Corp · Fall from machine

EE was knelt down on stinger conveyor greasing head pulley & checking drive belts on main conveyor. Another miner started the conveyor, causing EE to fall 12' off end of conveyor. Ambulance was called with previous back surgeries as a precaution. EE was taken to Hospital and released.
